Migliorare le cure palliative transnazionali attraverso la farmacogenetica e il monitoraggio terapeutico dei farmaci (CROSS-PaGe)
L'obiettivo generale del progetto CROSS-PaGe è stabilire le condizioni scientifiche, metodologiche e cliniche necessarie per "voltare pagina" nella gestione delle cure palliative nell'area transfrontaliera Italia-Austria. Il progetto mira a migliorare significativamente lo stato attuale dei trattamenti clinici e farmacologici negli ospedali di riferimento situati nelle regioni di confine attraverso l'introduzione di tecnologie avanzate come la farmacogenetica e il monitoraggio terapeutico dei farmaci (TDM).
La cooperazione transfrontaliera è essenziale per raggiungere questo obiettivo, poiché il livello di adozione e integrazione di queste tecnologie nelle cure palliative rimane molto frammentato nell'area del programma. Sebbene siano disponibili linee guida europee sulla farmacogenetica, esse vengono applicate in modo incoerente da un ospedale all'altro, portando a risultati terapeutici diseguali tra i pazienti palliativi.
Il progetto cerca inoltre di colmare il divario tra gli istituti di ricerca e le aziende biotecnologiche, che spesso sono all'origine di soluzioni diagnostiche innovative – come i kit per il test rapido del DNA – ma la cui adozione da parte delle strutture cliniche è limitata. Questa opportunità mancata di trasferimento dell'innovazione è una sfida chiave che CROSS-PaGe intende affrontare, facilitando la comunicazione, la co-progettazione e la validazione degli strumenti direttamente in ambito clinico.
Tutti i deliverable previsti dal progetto CROSS-PaGe sono stati concepiti per generare impatti concreti e duraturi sull’area di programma e sulla popolazione residente, con particolare riferimento al miglioramento dei percorsi di cura in ambito palliativo e all’introduzione sostenibile di strumenti di medicina personalizzata nei sistemi sanitari pubblici.
Il principale prodotto del progetto – il protocollo terapeutico transfrontaliero personalizzato, basato sull'integrazione tra farmacogenetica e TDM – sarà adottato in modo permanente dai partner clinici pubblici coinvolti (CRO Aviano, Burlo Garofolo, Bolzano, Klagenfurt), che lo integreranno nei propri standard di trattamento per i pazienti palliativi. L'inserimento del protocollo nei documenti operativi delle strutture sanitarie garantirà la sua applicazione sistematica anche dopo la chiusura del progetto.
Il programma di cooperazione Interreg Italia-Austria è cofinanziato dal Fondo Europeo di Sviluppo Regionale (FESR). Esso mira a creare sinergie e a trovare soluzioni comuni per superare le sfide e gli ostacoli transfrontalieri allo sviluppo dell'Italia e dell'Austria.

Improving Transnational Palliative Care through Pharmacogenetics and Therapeutic Drug Monitoring (CROSS-PaGe)
The overall objective of the CROSS-PaGe project is to establish the scientific, methodological, and clinical conditions necessary to “turn the page” in the management of palliative care in the Italy–Austria cross-border area. The project aims to significantly improve the current state of clinical and pharmacological treatments in reference hospitals located in border regions through the introduction of advanced technologies such as pharmacogenetics and therapeutic drug monitoring (TDM).
Cross-border cooperation is essential to achieve this objective, since the level of adoption and integration of these technologies in palliative care remains highly fragmented within the programme area. Although European pharmacogenetic guidelines are available, they are applied inconsistently from one hospital to another, leading to unequal therapeutic outcomes among palliative patients.The project also seeks to bridge the gap between research institutes and biotechnology companies, which are often the originators of innovative diagnostic solutions – such as rapid DNA testing kits – but whose adoption by clinical facilities is limited. This missed opportunity for innovation transfer is a key challenge that CROSS-PaGe intends to address by facilitating communication, co-design, and validation of tools directly within the clinical setting.
All deliverables envisaged by the CROSS-PaGe project have been designed to generate concrete and long-lasting impacts on the programme area and the resident population, with particular reference to improving palliative care pathways and the sustainable introduction of personalized medicine tools into public healthcare systems.The project’s main output – the personalized cross-border therapeutic protocol, based on the integration of pharmacogenetics and TDM – will be permanently adopted by the public clinical partners involved (CRO Aviano, Burlo Garofolo, Bolzano, Klagenfurt), which will integrate it into their treatment standards for palliative patients. The inclusion of the protocol in the operational documents of healthcare facilities will ensure its systematic application even after the project’s conclusion.

Verbesserung der grenzüberschreitenden Palliativversorgung durch Pharmakogenetik und Therapeutisches Drug Monitoring (CROSS-PaGe)
Das übergeordnete Ziel des Projekts CROSS-PaGe besteht darin, die wissenschaftlichen, methodischen und klinischen Voraussetzungen zu schaffen, die notwendig sind, um die Palliativversorgung im grenzüberschreitenden Raum Italien–Österreich grundlegend weiterzuentwickeln. Das Projekt zielt darauf ab, den aktuellen Stand der klinischen und pharmakologischen Behandlungen in Referenzkrankenhäusern der Grenzregionen durch die Einführung fortschrittlicher Technologien wie Pharmakogenetik und Therapeutisches Drug Monitoring (TDM) deutlich zu verbessern.
Die grenzüberschreitende Zusammenarbeit ist wesentlich, um dieses Ziel zu erreichen, da der Grad der Einführung und Integration dieser Technologien in der Palliativversorgung im Programmgebiet nach wie vor stark fragmentiert ist. Obwohl europäische Leitlinien zur Pharmakogenetik verfügbar sind, werden sie von Krankenhaus zu Krankenhaus unterschiedlich angewendet, was zu ungleichen therapeutischen Ergebnissen bei Palliativpatientinnen und -patienten führt.
Das Projekt verfolgt außerdem das Ziel, die Kluft zwischen Forschungseinrichtungen und Biotechnologieunternehmen zu überbrücken, die häufig innovative diagnostische Lösungen – wie Schnelltests für DNA-Analysen – entwickeln, deren Einführung in klinischen Einrichtungen jedoch begrenzt bleibt. Diese verpasste Chance des Innovationstransfers stellt eine zentrale Herausforderung dar, der sich CROSS-PaGe durch die Förderung von Kommunikation, Co-Design und Validierung der Instrumente direkt im klinischen Umfeld widmen möchte.
Alle im Rahmen des Projekts CROSS-PaGe vorgesehenen Deliverables wurden so konzipiert, dass sie konkrete und langfristige Auswirkungen auf das Programmgebiet und die dort lebende Bevölkerung erzielen, insbesondere im Hinblick auf die Verbesserung der Versorgungspfade in der Palliativmedizin sowie die nachhaltige Einführung personalisierter Medizin in öffentliche Gesundheitssysteme.
Das wichtigste Projektergebnis – das personalisierte grenzüberschreitende Therapieprotokoll, das auf der Integration von Pharmakogenetik und TDM basiert – wird dauerhaft von den beteiligten öffentlichen klinischen Partnern (CRO Aviano, Burlo Garofolo, Bozen, Klagenfurt) übernommen und in ihre Behandlungsstandards für Palliativpatientinnen und -patienten integriert. Die Aufnahme des Protokolls in die operativen Dokumente der Gesundheitseinrichtungen gewährleistet dessen systematische Anwendung auch nach Abschluss des Projekts.
Das Kooperationsprogramm Interreg Italien–Österreich wird durch den Europäischen Fonds für regionale Entwicklung (EFRE) kofinanziert. Ziel des Programms ist es, Synergien zu schaffen und gemeinsame Lösungen zu finden, um grenzüberschreitende Herausforderungen und Hindernisse für die Entwicklung Italiens und Österreichs zu überwinden.
